How to request additional boxes earned:

2. Submit form and invoice(s), showing the BD products purchased, by uploading and attaching to this form.
3. BD will mail the additional boxes earned. Customer should receive the product within 6 to 8 weeks.

Rules and eligibility:
1. Promotion runs from August 1, 2016 through November 30, 2016
2. Eligible product purchases can occur at once or anytime during the promotion period to receive additional boxes. Multiple invoices that add up to the requirement for total boxes purchased should be submitted at one time using a single claim form.
